• PIN for online registration can be purchased through online payment of Rs. 200/- from the website (www.medadmgujarat.org)
• After completing online registration, candidates must make prior appointments for the Document Verification and Submission of self-attested photocopies of Documents at the Help Centre.
• For Local Quota: Candidates who want to apply for the Local Quota Seats of Smt. NHL Municipal Medical College, Ahmedabad or Surat Municipal Institute of Medical Education & Research, Surat. Students must produce a resident certificate also
• Students need to submit a list of preferred colleges that includes top medical colleges of Gujarat, like the RUHS Medical College.
• For NRI Candidates only: After online application, they must submit a Demand draft of Rs. 10,000/-

• H.S.C. Standard 10th (SCC) mark-sheet

• Domicile certificate issued by the competent authority of the Government of Gujarat for Gujarat-based Candidates
• Certificate belonging to SC issued by the authority empowered by the Government of Gujarat for ST candidates.
• Defence certificate issued by the competent authority of the Government of Gujarat for children of defence personnel
